Is the program/event/clinic sponsored and operated by Tarleton using a Tarleton facility? Yes No
Is the program/event/clinic sponsored by a third party using a Tarleton facility?

Is the program/event/clinic sponsored by a third party using a Tarleton facility? Yes No
Is the program/event/clinic sponsored by Tarleton and located at a non-Tarleton owned or leased facility?

Is the program/event/clinic sponsored by Tarleton and located at a non-Tarleton owned or leased facility? Yes No
